New Patient Forms

Hello to Our New Patients!

For our patients’ convenience, we have uploaded our new patient documents to download, fill out and bring to your appointment. If you are unable to bring these documents with you, we ask that you arrive 15-20 minutes prior to your appointment to fill them out in the office. Please contact our office if you have any questions.

During your first visit, you can expect to stay with us for an hour and a half. During your appointment, you will meet our experienced team, review all of your questions and concerns with Dr. James and receive a tour of our state-of-the-art office. We will take X-rays, and perform a thorough exam and cleaning. At that time, if there is any recommended treatment, Dr. James will discuss your options and create a treatment plan suited for your individual needs.

Parking:

For your convenience, there is a parking lot directly behind the office, as well as covered parking underneath the Alexan West Dallas next door to us.

*** For areas west of Sylvan and Fort Worth Ave, take Fort Worth Ave. to Yorktown and turn right onto Yorktown, the parking lot will be the first driveway on your left hand side.

*** For areas east of the Trinity River, take either the Margaret Hunt Hill bridge or the Commerce Street Bridge west towards Trinity Groves. Commerce street becomes Fort Worth Ave. At Yorktown Street (just past Commerce Grinding) make a left turn. The parking lot will be the first driveway on your left hand side.
